Each individual applying for 10-point veteran preference should fill form sf 15 out. It is critically important to answer all the questions given and provide only truthful data. After it is completed, go to the next step. Each form should be supported by certain amount of documentation that will prove that you belong to an eligible category.
Pay attention to the information below.
1. You may prepare any documentation of service and separation under honorable conditions or Documents from the Armed Forces. These types are both taken into account. To verify your rights in this matter attach the copies of the following papers to your SF 15:
Certificate of Release or Discharge from Active Duty, DD-214. It is issued to militaries after the separation from active service. It includes information about the term of military service and date of the separation. Documents that confirm the transfer to Navy Fleet Reserve, Marine Corps Fleet Reserve, or enlisted Reserve Corps.
Orders of separation or retirement
Affirmation of being discharged or released not later than 120 days after the certification is signed.
Official statement from the Military Personnel Records Center that honorable separation took place.
2. If you want to demonstrate that you belong to the part of Documentation of service-connected disability, Purple heart or Nonservice-connected disability pension prepare next documents together with standard form 15:
An official statement, dated 1991 or later, from the Department of Veterans Affairs or from the Armed Forces, certifying to the present existence of the veteran’s service-connected disability of less than 10%.
A discharge certificate by an affiliate of the Armed Forces, that demonstrates the Purple Heart award to the veteran.
An official declaration, dated 1991 or later, from the Department of Veterans Affairs, verifying that the veteran is receiving a pension.
3. To prove your eligibility to obtain a veteran preference for the reason of service-connected compensable disability, prepare the next papers:
An official paper, dated 1991 or later, from the Department of Veterans Affairs or from the Armed Forces, which confirms that veteran receives compensation or pension.
A document, dated 1991 or later, from the Department of Veterans Affairs or from a branch of the Armed Forces, certifying that the veteran has a service-connected disability of 10%.
An official certificate from a branch of the Armed Forces, that demonstrates that the serviceman was retired because of permanent service-connected disablement incapacity. This paper should affirm that it is 10% or more.
4. Wives and mothers of veterans should submit a sf 15 form with an official statement, dated 1991 or later, from the Department of Veterans Affairs or from a branch of the Armed Forces to confirm the present veterans incapacity, the nature of the service-connected incapacity, a notation mentioning whether or not the veteran is currently unemployable due to the health problems, and a note about whether or not the service-connected disablement incapacity is permanent.
5. Documentation of veteran’s death is used veteran’s family and near relatives.
If death occurred not on active military duty, a person should submit an official copy of death certificate.
If death occurred on active military duty, the family receive a DD Form 1300, Report of Casualty to file instead of DD Form 214.
A person should prepare a documentation of service or death during a war, in an expedition or campaign during the period of April 28, 1952, till July 1, 1955.
6. If veteran’s mother is disabled it is possible to claim for preference for her because of her son’s disability by preparing an appropriate certificate.
The last part of SF 15 presupposes giving answers to certain questions about the veteran’s inability to work because of a service-connected disability.
